Brothers and Sisters,
In an effort to assure that employees receive the proper information when they put in for retirement, starting March 1, 2010 the employee will need to fill out the attached form when they give notice of their pending retirement. This is for all “NON-VERO “retirements.
All of our retirement requests will be processed through Syracuse and with number different practices within all the legacy companies it was apparent that there was a need to standardize the process. By utilizing this form it will help all parties involved better track the process and to make sure the employee receives all of the needed paper work and informational materials on time.
Once the employee has completed the form they need to turn it into their immediate supervisor for review to make sure all of the needed information is on the document. The supervisor does not make the determination if the retirement is granted or not. The supervisor will in turn forward the form to Syracuse.
